Join the NELFT trial and get ready for your Annual Health Check

Maldaba is very excited to be working with NELFT (North East London NHS Foundation Trust) to improve the lives of people with learning difficulties through use of the Hear Me Now app.

As part of the year-long project anyone registered at either Newbury Group Practice or Fullwell Cross Medical Centre as having a learning disability can take part. The project aims to make it easier and more engaging for people with LD to prepare for and complete their next Annual Health Check.

The surgeries have specially adapted easy-read digital pre-Annual Heath Check questionnaires that they can send app users individually, safely and securely so they can be completed quickly and efficiently ahead of their scheduled Annual Health Check. The GPs will be fully prepared ahead of the meeting so they can spend better quality time discussing any specific health concerns that are raised.

As well as helping people with their Annual Health Checks, Hear Me Now has proved to help transform the lives of people with all manner of cognitive impairments, reducing hospital visits and saving time and increasing engagement.
